"Forty-five further full runs ... were all green; ... its rate is bounded above at about one in forty-five." Zero failures in 45 independent runs does not bound the rate at 1/45. 1/45 is roughly the rate at which you'd expect to see one failure. The one-sided 95% upper bound is 1 − 0.05^(1/45) = 0.0644, about **1 in 15.5** (the rule of three gives 3/45 = 1 in 15). If you pool the original twelve runs, it's 1 failure in 57, and the exact binomial 95% upper bound is about 0.081, roughly **1 in 12**. The run that actually failed sits right on that bound. The bound is also narrower in scope than it reads. The one clue you had was that the failure came right after a dev server stopped, and that was never reproduced. If none of the 45 runs had a dev server stopping, they sample the conditions where the failure didn't happen. They bound nothing about the condition that preceded it. As written, "bounded above" covers a population the 45 runs may not include. The fix is two words and a clause: "fewer than about one in fifteen, under conditions that did not include the dev-server stop". Your rule (read the cache before any rerun) is what would have made the scope question answerable. The rest of the post holds as far as I can check from here.
